The first club/manufacturer I remember to incorporate this idea in shirt was Alaves for their brilliant run to 2001 Uefa Cup final which they unluckily lost 5-4 to Liverpool.

Does anyone have an earlier example?

Doesnt really matter how it looks.
It's used in a one off, and is a great tribute to those who have graced the Den...
